Pathogenesis and treatment of renal osteodystrophy.

Renal osteodystrophy is the term used to describe the many different patterns of the skeletal abnormalities that occur in patients with chronic kidney disease. The main two conditions are osteitis fibrosa, characterized by high bone turnover, increased osteoclastic and osteoblastic activity, and high levels of circulating parathyroid hormone (PTH) and adynamic bone disease characterized by low ...

Alkali treatment of renal osteodystrophy.

In 1960 a 5-year-old girl was found to have developed accelerated hypertension (B.P. 190/150) with blood urea 119 mg/ 100 ml. An intravenous pyelogram showed a small scarred left kidney (5-3 by 32 cm). There was also upper pole scarring with clubbed calyces on the right side. Left nephrectomy was performed and histological examination of the kidney confirmed pyelonephritic changes, and showed i...

Renal osteodystrophy.

The incidence of chronic renal disease is increasing, and the pattern of renal osteodystrophy seems to be shifting from the classic hyperparathyroid presentation to one of low bone turnover. Patients with persistent disease also live longer than previously and are more physically active. Thus, patients may experience trauma as a direct result of increased physical activity in a setting of weake...

[Renal osteodystrophy and secondary hyperparathyroidism].

High-turnover bone due to excess PTH is one of the main types of renal osteodystrophy. In addition to the classic stimuli of PTH secretion such as hypocalcemia and decreased production of active vitamin D, new mechanisms has been suggested. Furthermore, skeletal resistance to PTH, a classic stimulus of PTH secretion in uremia, has been recently revisited as an underlying abnormality.
